State of CT requirements regarding forms:

The state requires that each line item on all forms (including the questionnaire) is filled out. Therefore, rather than leaving a line blank, please write “N/A (Not Applicable)” or “Same” as appropriate.

A student’s health form must list a physical exam date that is within twelve months of the school start date. Updated health forms must be submitted when the physical exam date expires, usually on or near your child’s birthday.

For example: Justin’s health form lists a physical exam date of March 19, 2018 (near his fourth birthday.) This exam will be valid at the start of school in September 2018, but will expire on March 19, 2019. At this time, a new health form which shows an updated physical exam must be submitted for Justin to stay enrolled at school.
